Done - let me know if anything breaks.

Just a heads-up -- tomorrow I will commit a change to MixerChannel to remove the postSendReady flag. I put that in initially as an optimization, but in hindsight, the benefits are microscopic and the level of confusion that the flag introduces is most unwelcome.

You should not notice any change in behavior, except in the following cases:

- If you create MixerChannels and address the postSendReady argument using a keyword, e.g. MixerChannel(\dummy, s, 1, 2, postSendReady: true). The interpreter will post a warning that the keyword was not found, but the mixer should be created normally anyway and it should work. (It isn't a fatal error because the argument would be ignored even if I left it in.)

- If you create your own MixerChannelDefs -- currently you must supply two synthdefs. After the change, only one will be required. The help file will show the new format (which is the same as the old format, minus the "basicFader" synthdef).

I apologize in advance if this breaks anyone's code, but it's worth it for simplicity.
